Reproduce below the sources of revenue and the amounts specified for each as identified in your organization's financial statements. Select whether each item that is a source of revenue received as a result of fund raising efforts (e.g., contributions, grants, bequests, etc.) or is revenue that is not related to fund raising (e.g. investment income, rental income, realized and unrealized gains or losses, etc.).

If using Audited Financial Statements, the sources of revenue can be found on the Statement of Activities page. 

If using the IRS Form 990, the sources of revenue can be found in Part VIII: Statement of Revenue (generally located on page 9).
